Croup, The On-The-Spot Diagnosis

General

Cough is a common symptom of many diseases. In this article, I’m going to talk about a specific cough caused mostly by viruses. It is called the viral croup.

What is croup?

In this disease, viruses attack the larynx, [voice box] the trachea, [windpipe] and sometimes the bronchus. A characteristic sign [dog-like] cough with basic loss of voice – hoarseness or [croupy – voice]constitutes a clinical diagnosis of croup.

Depending on the parts of the airways involved, croup is also called laryngotracheitis or laryngotracheobronchitis.

Viral Croup is confirmed on the spot

Excellent doctors diagnose ‘on the spot’ all the time. A six-year-old boy in Nigeria, who has been experiencing fever and chills, has malaria until proven otherwise. A 3-year-old girl, who can’t raise her hand after being forcefully pulled by a caregiver, suffered from a condition known as nurse’s elbow. Croup is one of those conditions where clinicians can make an ‘on the spot’ diagnosis

A week ago, I heard a child in the waiting room coughing. It was a hacking cough. Before I saw that child, I had a diagnosis of croup in mind.

Skin signs and symptoms

Viral causes of scabies

Of the viruses, Parainfluenza virus is often responsible. Influenza virus, respiratory influenza virus and adenoviruses are some of the other viruses that cause croup. Measles virus is a major cause of severe scabies in developing countries. The viruses spread from one person to another through sneezing and coughing.

When viruses attack the inner lines, the epithelium of the upper windpipe; the larynx, the trachea, and the bronchus, they promote swelling, inflammation and edema. This leads to narrowing of the lumen of the upper airways.

Research

None are needed when the clinical diagnosis is simple and the child responds to management. If a clinician for some reason decides to get an x-ray of the neck, it will show a narrowing of the lumen of the upper part of the trachea, also known as the steeple sign.

Handling

You will meet many children with moderate to severe croup in the emergency room.

Physicians and other practitioners know not to aggravate children with croup. These children will remain in the comfortable arms of their mothers while they are assessed and treated.

Some clinicians will begin with a gentle cool mist near the child’s face while definitive treatment is on the way.

There are two popular remedies; nebulized use [inhaled] racemic epinephrine and a shot or oral administration of a steroid. Both treatments reduce edema around the windpipe and work to open it up so more air can pass through.

When the treatment is successful, the clinical result is amazing. Parents are relieved when they notice that the whooping cough is gone and the child is less anxious.

A dexamethasone shot is the drug of choice.

An oral dose of Dexamethasone is equally effective.

Prednisolone is given for a day or two in places where dexamethasone is not readily available.

Another treatment in croup is racemic epinephrine, given as a nebulized solution.

Children who have done well on epinephrine still need to be watched by health care providers for at least two hours. This is because when the effect of epinephrine wears off, the airways may constrict again, known as rebound airway obstruction. Symptoms of the rebound effect of epinephrine include the return of croupy cough and resumption of difficulty breathing.

Humidifier

For a very mild cough, the humidity of the air in the room can help soothe an inflamed throat and may speed secretions. There is no need to add menthol or other substances to a humidifier.

Problems

A crust will dissolve over several days. Before settling, crab can create fertile ground for more serious diseases. An ear infection can develop as an extension of croup. Pneumonia and bacterial tracheal infection are other diseases that can complicate croup.
