Hair Follicle Cloning – A Summary of the Recent Literature as Related to the Treatment of Hair Loss

There are many limitations to the cosmetic, medical and surgical treatments currently used to treat hair loss. Eventually, newer treatments will emerge that may have the potential to permanently “cure” congenital baldness. These techniques include the concepts of hair follicle cloning and gene therapy.

Hair follicles are miniature hair growth organs that develop through cycles of growth and rest. In addition to growing and then shedding hairs during these phases, the follicle itself breaks down almost completely by the end of the regression phase, and an almost entirely new follicle is formed at the beginning of the next growth phase. It provides a unique opportunity to use advanced molecular biological and medical techniques leading to cloning and gene therapy.

Cell Biology and Genetics

Cells are the basic unit of all living organisms. A hair follicle is a miniature organ that contains several different types of cells that work together to grow hair. Inside each cell is a NUCLEUS that contains CHROMOSOMES, which are made up of DNA, the cell’s genetic material. Genes are sections of DNA that code for specific types of proteins. Proteins, on the other hand, determine the actual characteristics, such as hair color, eye color, baldness, etc.

Each cell of a multicellular organism contains in its chromosomes a COMPLETE DNA BLUEPRINT of all genes proteins for the entire organism. Unlocking this DNA information in mature specialized cells is an important aspect of some cloning techniques.

Cell replication

In a rapidly growing embryo, cells replicate by splitting in half and then growing back to full size. This process is called MITOSIS of cells. Each half of the cell that divides contains a complete and accurate set of the organism’s DNA. As the embryo becomes a more complete organism, its cells begin to acquire more specialized characteristics and begin to divide less. As cells become more specialized, cell replication shifts to more specialized cells called stem cells. As specialized cells wear out over time, they must be replaced from this pool of stem cells, which can give rise to many different types of specialized cells.

CLONING

The cell biology background described above forms the scientific basis of cloning. Cloning is defined as the creation of an EXACT GENETIC REPLICA BY Asexual MEANS. The use of fetal or embryonic tissue is an integral part of this technology. Scientists CLONE a gene, a cell, or an entire organism.

Cloning as defined above must be distinguished from TISSUE ENGINEERING or CELL THERAPY. Tissue engineering involves culturing cells in vitro and re-implanting them. An example of such tissue that could potentially be cultured and implanted is hair follicles. Tissues such as hair follicles are not called clones.

However, in the imperfect world, the following terms have become interchangeable: hair follicle cloning, follicular cell implantation, follicular neogenesis, follicular regeneration, and hair propagation. However, according to scientific protocol, the first term, cloning, should be excluded from the list because the technique described here is based on tissue engineering.

Why then does research continue in this direction? What is the driving force and/or rationale for tissue engineering or cloning? First, there is the promise of UNLIMITED DONOR HAIR. There have been countless occasions when the patient has been deemed unsuitable for surgical hair restoration due to the rarity and quality of the donor hair. This would never happen given an unlimited number of donors. A second driver would be LACK OF DONOR SCAR. This would please many patients and doctors alike. GENERATION OF HIGHER DENSITY and EASIER PERMISSION OF THE PROCEDURE also make the concept of cloning attractive to clinicians.

CONCEPT

In theory, the tissue engineering concept would involve harvesting a small sample of hair follicles from the same donor region used in the hair transplant. Follicle-inducing cells in the hair follicle would be isolated and then induced to multiply using cell culture. The newly multiplied cells would then be re-implanted in the recipient area of ​​the scalp to create new follicles. Due to the cell division that occurs during cultivation, countless new cells are created which, when implanted, create hundreds of NEW HAIR FOLLICLES.

JUSTIFICATION

Follicular cell implantation (FCI) was based on fundamental research into normal hair growth. Hair growth occurs as a result of the DYNAMIC INTERACTION of the components of the epidermis and dermis. This DERMAL AND EPIDERMAL INTERACTION determines the development of fetal follicles as well as the normal formation of hair shafts during anagen.

Colin Jahoda demonstrated the validity of this interaction by taking rat whisker dermal papilla cells, growing these cells in culture, and implanting the cultured cells into rat incisional skin wounds. The implanted cells induced hair growth by interacting with local epithelial cells to regenerate hair follicles and form the hair shaft. This is similar to what happens during Fetal DEVELOPMENT of hair follicles and the normal ANAGEN PHASE of the growth cycle. Several investigators, including Jahoda, Cooley and Vogel, Unger and the Aderan Research Institute, have replicated this work in humans. The problem is that nothing has appeared in the way of detailed research to describe the actual TECHNIQUES used in these human studies. Because successful research has commercial value, the importance of intellectual property protection has overshadowed the incentive to publish.

APPLICATIONS

The use of TISSUE ENGINEERED cells to treat hair loss is conceptually quite simple, but many complexities and challenges make this application uncertain. Research efforts have spanned more than twenty years, but results have been mixed at best. This points to various obstacles that still need to be overcome. The main problems are:

(1) Autologous vs. allogeneic tissue: must the tissue used be from the individual undergoing the procedure, or can tissue from another individual be used to create the engineered follicle? Some parts of the hair follicle seem to be immunologically “privileged”, but it is currently unknown whether allogeneic tissue will work.

(2) Which follicular cells should be cultured and implanted? Controversy exists about the type of cells to be cultured and implanted. Skin cells, epidermal cells, stem cells, and germinal epithelial cells may have the potential to form new hair follicles in culture, but there is no consensus in the research literature.

(3) The ability to maintain the inductive potential is an important factor in the implantation of tissue-engineered follicles. After several passages in culture, papilla cells lose their ability to induce hair growth when reimplanted. This could be a major obstacle to the development of hair loss treatments.

(4) The cosmetic properties of the resulting cloned hair will be important. Hair color, orientation, curl and caliber will need to be analyzed to achieve a “normal” looking result.

(5) There are economic and regulatory hurdles to overcome before FCI can be considered as a treatment for hair loss. In the United States, the US Food and Drug Administration will likely regulate implanted hair cells as a “biological therapy.” This means a comprehensive regulatory framework and significant legal barriers that could hinder or delay FCI as a hair loss therapy.

(6) Finally, there is the matter of safety. Whether FCI can cause tumorigenesis needs to be investigated. The issue of infectious disease transmission, especially when allogeneic tissue is used, will become very important in FCI therapy.

SUMMARY

FCI is able to overcome many limitations of current surgical hair restoration, especially the limited amount of donor hair. The basic concept is sound, but human reports show inconsistencies and reproducibility issues. The prospect of an unlimited supply of donors will continue to influence tissue engineering research to overcome these obstacles.
